Responsibilities

Hixson- Part time- 1200-2200

The Clinical Staff Pharmacist is a member of the multidisciplinary care team, providing clinical pharmacy services to
patients including pharmacokinetic consultations, total parenteral nutrition consultations, anticoagulation management,
antimicrobial stewardship, medication reconciliation, discharge counseling, formulary management, and compliance
with clinical pathways and protocols. The Clinical Staff Pharmacist will work centrally and de-centrally to perform all
functions of a clinical pharmacist including accurate dispensing, clinical rounding, including patient involvement, and
supervision of the technicians. The Clinical Staff Pharmacist will serve as a resource to other health care professionals
through education and training programs. The Clinical Staff Pharmacist will apply clinical systems technology and
distribution automation systems to provide effective, high quality, safe patient care.

ESSENTIAL KEY JOB RESPONSIBILITIES
1. Participate in all aspects of CHI Memorial's comprehensive clinical pharmacy services including rounds with
medical teams, pharmacokinetic consultations, parenteral nutrition consultations, anticoagulation management,
antimicrobial stewardship monitoring, medication reconciliation, discharge counseling, documentation of clinical
interventions in the identified data base, and compliance with clinical pathways and protocols
2. Process prescriber orders in the identified pharmacy information systems in an effective, efficient, timely, safe
and accurate manner to ensure the appropriateness of the indication, dose/frequency, route, interactions, dose
adjustment, physical and therapeutic incompatibilities, side effects, duplicate therapies, and cost-effectiveness.
3. Evaluate need for all non-formulary medication required.
4. Provide direct oversight and timely and accurate verification of final product for all aseptic parenteral and non-
parenteral medication preparation, dispensing and distribution in compliance with state, federal statues and
hospital procedures.
5. Ensure doses are filled and delivered within established departmental guidelines in a timely and safe manner.
6. Identify and evaluate adverse drug reactions and medication errors
7. Provide enhanced communication and drug information to healthcare workers and patients
8. Provide teaching, supervision, and serve as a role model to technicians, residents, and students

9. Participate in professional activities within the department and in the community
10. Other duties as assigned by management

Qualifications

Bachelor's Degree and minimum of 1 year experience in the discipline OR 3 years' experience in the discipline OR Master's Degree and no experience

Graduate of an Accreditation Council for Pharmacy Education (ACPE) approved College of Pharmacy with a BS, MS, or Pharm D degree, including didactic courses and experiential rotations in the clinical aspects of
pharmaceutical care.

Completion of an ASHP-accredited PGY-1 residency program or equivalent clinical experience.
