Bloomfield West, a 59-bed ICF-DD facility, is looking for a Physical Therapist to work with intellectually disabled residents. Here are some of the responsibilities required:
- Provide assessments and evaluation of residents identified to have physical therapy needs by the interdisclipinary team (IDT).
- Participation in the continuing IDT evaluation of individual resident's progress.
- Face to face interaction with residents and program staff to monitor progress as needed.
- Annual and semi-annual review of the physical therapy program for each resident and recording of such.
- Provision of in-services to all staff as requested by the QIDP and Administrator on physical therapy needs of residents.
