Gurwin Certified Home Care Agency is looking for Fee For Service Physical Therapist to cover Western Suffolk/North Shore.

Gurwin Healthcare System is a full continuum of care providing extraordinary care and caring to the people that we serve. Set on a 34-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis. Sharing the campus is Gurwin's Assisted Living community (Gurwin Jewish ~ Fay J. Lindner Residences), our independent living community (Fountaingate Gardens) and our Certified and Licensed Home Care Agencies.

Our innovative philosophy and growing services offer opportunities to advance your career. We are currently seeking a Physical Therapist for our Home Care Division with adult experience who will embody the passion and commitment required to deliver the ultimate care for our patients.

EDUCATION AND EXPERIENCE REQUIREMENTS

A current New York State Physical Therapy License.

Graduate from a physical therapy program approved by the American Physical Therapy Associate or the American Medical Association.

Minimum of one year experience as a physical therapist.

Home health experience is preferred.

Comfortable with laptop/tablet and integrating them into a home visit.

PHYSICAL WORK REQUIREMENTS:

Ability to travel to patients' homes to make home visits.

Ability to make clinical and managerial decisions within the scope of professional practice to perform physical therapy procedures competently and independently.

RESPONSIBILITIES INCLUDE, BUT NOT LIMITED TO:

Reports the patient's initial status, significant changes, therapy plans and goals, on-going status, and discharge plans.

Provides the Agency with timely documentation including, but not limited to, assessments, plans, goals, visit care, summary reports, and billing/visit data.

Instructs patients, families and team members regarding programs that enhance and/or facilitate care and follow-up.

Evaluates the patient's social and architectural environment, makes recommendations, and implements them, as feasible.

Complies with all regulatory criteria.

Notifies the agency of visit schedule and changes.

Provides professional leadership and clinical guidance and expertise to team members.

Attends regular continuing education programs.

Performs related duties, as necessary.
