Penn Home Infusion Therapy, certified by the Joint Commission, provides patients with necessary treatment and intravenous medication in the comfort of their own home, which can lead to more flexibility and independence in their daily routines. Our infusion services are available in Pennsylvania, southern New Jersey, and northern Delaware, and our multidisciplinary team approach allows us to provide continuity of care from hospital to home.
Our nurses and therapists undergo extensive home health care training, an annual competencies review, as well as professional development, a clinical advancement program and lifelong learning on the most cutting-edge clinical procedures. With an in-house education department, we bring a dedication to educational preeminence and exceptional quality.

Follow Up Care a Top Priority
Although rehabilitation at home is more comfortable and less stressful than rehabilitation in a hospital or outpatient facility, we understand patients needing care may still have questions and concerns. Our staff will contact a referred patient as soon as possible to schedule an appointment and make sure all their questions are answered.
Patient Education and Empowerment
Penn Medicine Home Health care teams provide patients and their caregivers with the tools they need to perform rehabilitative procedures correctly, make appropriate lifestyle changes and provide proper care for their loved one. Our goal is to help patients recover as safely and quickly as possible, supporting independence as their conditions improve.
Additionally, our home health experts focus on medication education and adherence, promoting safety in the home and limiting unnecessary hospital and emergency room visits. We focus on each patient's individual care plan to ensure medication compliance. Our team partners with the patient and their doctor to make sure the plan of care is efficiently implemented, expediting patient recovery and ensuring safety.
Qualifications:
Physical Therapist:
Bachelors Degree (Required)
Currently licensed as a physical therapist in Pennsylvania or NJ (Required)
Basic Cardiac Life Support (Required)
Car Insurance (Required)
Driver's License (Required)
Occupational Therapist:
Bachelors Degree (Required)
Currently licensed as a occupational therapist in Pennsylvania or NJ (Required)
