Schedule: PRN, as needed

Shift : Work a minimum 4 days a month, 2 of which days must be weekend days.

Location: This position must be willing to do home visits throughout the Pasco County area

The role you'll contribute:

Executes physicians order, and notifies physicians of necessary updates or changes in a home health patient's condition. Assesses, plans, implements, and evaluates the individualized physical therapy care provided to a patient. Responsible for the coordination of care of all assigned patients. Delivers high quality and professional physical therapy care; performs technical physical therapy skills with a high degree of proficiency, and promotes the team approach in the delivery of patient care. Completes documentation according to Agency Policy and Regulatory guidelines, and performs home visits as ordered by the patient's physician.

  • Performs comprehensive initial evaluation on all patients assigned requiring physical therapy.
  • Perform comprehensive home safety evaluation and communicates specific needs to the physician and the appropriate team member.
  • Establish a plan of care with goals based on the patient's current level of functioning and potential for improvement in conjunction with the patient/family/caregiver and in accordance with the physician's orders
  • Implement the physical therapy assessment and care plan, continuously evaluating the patient's response to program and need for modification.
  • Report pertinent observations to attending physician; changing patient care plan as directed.


Qualifications
The expertise and experiences you'll need to succeed:

Minimum qualifications :
  • Current PT licensure within the state of FL
  • Current BCLS
  • Valid Driver's License
  • BS in Physical Therapy by an American Physical Therapy Association accredited university.
  • Minimum of (1) year of clinical Physical Therapy in an acute or Rehab setting

Preferred qualifications:
  • MS or DPT in Physical Therapy by an American Physical Therapy Associate accredited university.
  • Two (2) years of Home Health Physical Therapy
