Responsibilities:
- Research requirements for program initiatives and manage physician practice participation for programs that include but are not limited to NCQA PCMH, and Commercial Payer incentive programs. Provide guidance, and support to ambulatory quality programs, and value-based initiatives.
- Manage the transformation of all primary care sites to achieve and maintain the most current Patient Centered Medical Home Certification through the National Committee for Quality Assurance (NCQA).
- Using a standardized tool, conduct PCMH assessments to assess a practice's current operations and procedures against the NCQA PCMH requirements.Prepare customized work plan for practice based on GAPS identified during assessments.
- Work with assigned practices on a regular basis to implement the customized work plan.
- Develop guidelines, protocols, and processes to ensure ongoing clinical quality improvement.
- Collaborate with physician practice leadership on evidence-based guidelines, workflow, and protocol development.
- Drive clinical efficiency and reduced variation across sites of care.
- Observe, identify, create, document, and facilitate workflow, redesign, and best practices.
- Track patients for specific chronic disease (or wellness) based guidelines across populations and monitor outcomes for specific interventions.
- Identify and develop suggestions, education, and training for provider improvement plans .
- Educate current and newly on-boarded staff on clinical quality measures, obtainment, and documentation.

- Clarify NCQA PCMH requirements and provide recommendations on how to meet the requirements,
- Connect practices with relevant educational resources, tools, and templates to meet the PCMH requirements.
- Identify challenges and barriers that may prevent practice from achieving goal in desired timeframe and communicating those challenges to Leadership.
- Facilitate brainstorming sessions/discussions among the team members.
- Facilitate and conduct abstracting and collection of both clinical and other pertinent data from the various insurance companies, electronic medical records, as well as quality metric systems to identify and prioritize opportunities to close gaps in care and enhance the Medical Group's ability to meet incentive and quality outcome targets through enhanced patient outcomes. Validate data collection to maintain data integrity. Utilize technology to close gaps through data submission, patient outreach, and clinical decision support.
Education, Training & Experience:
- Associate degree in nursing required, bachelor's degree preferred.
- Knowledge of the PCMH concept and the NCQA program preferred.
- Experience working with electronic health records.
- Strong communication and conflict resolution skills.
