Summary

The Community Outreach Coordinator is responsible for outreach events to engage individuals who would benefit from linkage to care and/or navigation within care and/or supportive services. Responsibilities include patient and community relations, volunteer oversight, safety, planning new events, and helping with developing event budgets. This position will be a liaison with community partners of the Ponce Center. The position will also directly assist existing Ponce Center patients with non-clinical patient navigation and linkage services.

Qualifications:

  • Three (3) or more years job related experience including managing outreach events and linking individuals to resources and/or services, customer service, health care, HIV/AIDS care and/or prevention.
  • Outstanding written communication and presentation skills.
  • Strong experience preparing reports.
  • Friendly, enthusiastic, and positive attitude.
  • Detail-orientated with the ability to manage multiple projects at a time.
  • Ability to build and maintain positive relationships.
  • Flexibility to work various shifts.
  • Bi-lingual in Spanish
  • Three (3) or more years job related experience including managing outreach events and linking individuals to resources and/or services, customer service, health care, HIV/AIDS care and/or prevention.


Preferred:

Bachelors and/or equivalent work experience.
