The Virginia Department of Health (VDH) is dedicated to protecting and promoting the health of Virginians. The VDH is made up of a statewide Central Office in Richmond and 35 local health districts.

Who We Are: The Roanoke City and Alleghany Health Districts (RCAHD) are two health districts in the Commonwealth of Virginia under the Virginia Department of Health and employ approximately 140 staff. RCAHD encompasses the cities of Covington, Roanoke, and Salem and the counties of Alleghany, Botetourt, Craig, and Roanoke.

The Population Health Planning & Improvement Coordinator provides or coordinates health improvement planning services in the local health district by planning, implementing, and evaluating health education activities. Coordinates activities including but not limited to: fulfilling health improvement and goals; preparing and submitting grant applications related to health improvement projects; delivering educational programs to clients and staff and conducting health education trainings; revising and developing educational materials; collecting program specific data and writing reports; conducting needs assessments and facilitating the CHIP process; providing technical assistance to local and community agencies/organizations regarding program planning; serving as public contact or PIO about services and activities and preparing news releases and coordinating district media contacts and activities. May include the supervision of staff. Serves as a member of the district's Management Team working closely with Health Director and Team to achieve VDH mission.

Duties Include:
*Managing CHA Coalitions with meetings facilitation and agenda development
*Analyzing data to drive community prioritization and response
*Incorporating health equity work into the Community Health Assessment
*Overseeing tow Staff-Health Equity Specialist and Community Health Epidemiologist
*Travel regularly to meetings throughout the health district to engage with community members and partners
*Drive population health strategies, including the Community Health Assessment (CHA) and Community Healt Improvement Plan (CHIP) and associated coalition development, programs, grant writing, and partnership development in rural communities

VDH requires all Health-Care Personnel (HCP) to provide documentation of immunization or evidence of immunity to the following: hepatitis B, measles, mumps, rubella, and varicella; a one-time dose of tetanus, diphtheria, and pertussis (Tdap) vaccine regardless of the interval since the last dose of tetanus-diphtheria (Td) vaccine was administered (Td or Tdap boosters shall be received every 10 years); and the current year's seasonal influenza vaccination.

Minimum Qualifications

• Knowledge of principles and practices of public health education; strategic and operational planning, budgeting, and operations; information research, quantitative methods for assessing effectiveness; community assessment & resources; group dynamics and behavior; population-based health strategies; health care practices & principles; human behavior and techniques for effecting behavior change; and grants management
• Skill in health program planning, implementation, and evaluation; development of goals and objectives; community assessment; technical and grant writing skills; public speaking and making presentations to staff and public; meeting and time management and organizing; and in the use of standard MS products, presentation, and desktop publishing software
• Ability to prepare news releases, brochures, teaching materials and attractive, informative displays for health fairs and other public presentations; seek out and interpret RFPs and grant offerings, create successful grant applications, and manage grants; work independently and as a team member; and create and manage effective interpersonal and interorganizational communication and coordination networks

Additional Considerations

• Previous experience in health promotion and media relations is preferred
• Certification as Health Education Specialist
