Primary Care Physician

Hartford HealthCare Medical Group, Inc. (HHCMG) seeks a full-time Primary Care Physician with the following job duties: Works in Primary Care Office (PCO) setting and in hospital/inpatient setting. Performs comprehensive assessments, treatment, and symptom and disease management of adult patients with a variety of medical complaints and diagnoses; provides patient and family education and support; documents patient information in the electronic patient record according to medical group policy and regulatory requirements; participates in quality improvement and practice development programs for PCO and hospital; provides leadership in committees and task forces and participates in educational activities related to hospital and primary care medicine services; admits patients in accordance with hospital patient acceptance policies; works with Emergency Department physicians and other specialty services to assure effective communication and transition of patients to optimize their care; works with other specialty services to assure highest quality of patient care through consultations to and from specialty services; and makes referrals to community physicians. Works at HHCMG's Primary Care Office, 1000 East Main Street, Torrington, CT 06790 and Charlotte Hungerford Hospital, 540 Litchfield Street, Torrington, CT 06790.

Qualifications:

The minimum requirements for the Primary Care Physician position are: U.S. M.D. or D.O. degree or the foreign equivalent; completion of three-year residency training program in Internal Medicine or Family Medicine; board eligibility for or certification in Internal Medicine or in Family Medicine; and possession of or eligibility for an unrestricted Connecticut medical license.
