Job Description

PURPOSE OF POSITION
NYC Health + Hospitals/Gotham Health, Gouverneur is recruiting a primary care physician (PCP) to provide Primary Care service to a panel of patients from infancy through age 22 years. This will include diagnosing and treating common Pediatric conditions, education for patients in developing skills and knowledge to control chronic illness, prevent illness through lifestyle modification and participation in recommended preventive services. PCPs perform diagnostic tests, as ordered, ensure patient/provider continuity and access to timely and appropriate levels of care. PCPs coordinate and manage their assigned panel of patients' care across various levels of service (emergency, primary, specialty clinic, dental, mental health, substance abuse, inpatient home care, and all other covered services). Where required by the practice, they are responsible for the supervision of mid-level providers.
Primary care physicians are the mainstay of the medical community because they provide the majority of bulk medical services to patients. They normally work as part of a group or individually. They are responsible for providing preventive care to their patients, which may include carrying out examinations on them on a regular basis.
The duties and responsibilities for this group of full-time providers are partially dictated by the NYS Medicaid Managed Care Program (NYS MMCP) requirements.

The Gouverneur Department of Pediatrics is a vibrant, mid-sized practice serving a diverse population of patients from throughout New York City. We are an active participant in the novel NYC H+H 3-2-1 IMPACT program which provides additional, multidisciplinary supports to families with young children beginning in the prenatal period. We are frequently innovators in Pediatric service delivery and clinical operations within the NYC H+H system. The department hopes to recruit an energetic Physician with enthusiasm for continuous innovation and performance improvement within the public sector.

AREAS OF RESPONSIBILITIES

 Prescribe or administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, or injury.
 Order, perform, and interpret tests and analyze records, reports, and examination information to diagnose patient conditions.
 Collect, record, and maintain patient information, such as medical history, reports, and examination results.
 Maintain EMR documentation according to Department standards.
 Monitor patients' conditions and progress and reevaluate treatments as necessary.
 Explain procedures and discuss test results or prescribed treatments with patients.
 Advise patients and community members concerning diet, activity, hygiene, and disease prevention.
 Refer patients to medical specialists or other practitioners when necessary.
 Coordinate work with nurses, social workers, pharmacists, psychologists, and other health care providers.
 Direct and coordinate activities of nurses, students, assistants, specialists, therapists, and other medical staff.

SUMMARY OF DUTIES AND RESPONSIBILITIES
1. Provides comprehensive primary care to all clinic pediatric patients from birth through age 22 years.
2. Manages both acute and chronic conditions.
3. Involved in the admission and discharge process of all their patients.
4. Reviews and updates the immunization status of all pediatric patients.
5. Performs preventive health services for all pediatric patients by schedule according to the American Academy of Pediatrics Standards.
6. Offers counseling to adolescent patients on substance abuse, sex education, HIV prevention, pregnancy prevention and tobacco counseling.
7. Provides developmental and anticipatory guidance to all patients as appropriate.
8. Reviews and manages all abnormal laboratory results and other tests.
9. Ensures that all patients with missed appointments are recalled in a timely fashion.
10. Ensures the safety of all patients while they are in the clinic setting.
11. Review and ensures that the National Patient Safety Goals are met at all times.
12. Ensures that allergies are properly documented on all patients.
13. Ensures that medications prescribed are fully explained to patients with the potential side-effects.
14. Required to complete annual evaluations.
15. Participate in committees as designated by your Medical Director.
16. Able to work evenings and weekends.
17. Direct participation in the 24-hour phone coverage for the Pediatric Primary Care Practice.
18. May be asked to move between sites as designated by Chief Medical Officer.
19. Peer review.
20. Attend team meetings and conferences.
21. Refer to specialties.
22. Works to close Care Gaps/improve Care Gap metrics.
23. Meeting clinical metrics.
24. Participates in departmental and facility Performance Improvement/Quality Improvement activities.
25. Timely closure of encounters (24 hours).
26. Time completion of patient forms.
27. Timely completion of work ques in EPIC.
28. Precepts School Based Health Centers (SBHC) providers as required.

Minimum Qualifications
1. Graduation from an approved Medical School; and,
2. Licensed to practice in New York State; and,
3. Completion of an approved residency in the specialty and Board Certified or Eligible (if Eligible, must obtain certification within five years of eligibility); and,
4. Must meet the criteria for credentialing and privileging in the department as defined by the Medical Staff By-Laws of both the facility employing the Physician and the back-up hospital, if these are different.
5. Must meet all NYS Medicaid Managed Care requirements.

MD/DO from an approved Medical School Completion of an approved Residency in Pediatrics

KNOWLEDGEABLE IN:

Spanish language skills preferred.

Ability to communicate effectively, both orally and in writing.

Ability to lead and train staff and/or students. Ability to analyze and solve problems.

Word processing and/or data entry skills.

EQUIPMENT/MACHINES OPERATED:

Computer, telephone, otoscope, ophthalmoscope,

Bilicheck, and other medical equipment.
