We're actively seeking a Primary Care Physician Lead who wants to make a difference in the lives of individuals and families struggling with mental illness. Under the clinical and administrative supervision of the VP of Medical Mental Health Services, the Lead PCP provides clinical care to patients with identified physical health conditions needing medical intervention. The PCP Lead will consult with the program psychiatrist and/or nursing staff regarding patients with a history of medical issues, or who are determined to have a medical condition needing further evaluation and/or treatment. The PCP will evaluate, diagnose, treat, and/or refer the patient to higher levels of medical care as clinically indicated. More specific administrative duties in this position are training and mentoring providers, including physicians and advanced practitioners. - to achieve performance and quality measures, develop strategies to improve communications with other medical and non-medical departments, and help manage day-to-day operations. The lead PCP will also develop, implement, and review clinical policies and protocols, conduct peer reviews, and collaborate with Administrators to ensure strategic goals are achieved. ( 80% of the time will be spent on direct patient care, and 20% of the time will be spent on administrative duties) .

Your Role in Action:
  • Works collaboratively with the program psychiatrists to identify and treat program patients for primary care medical services.
  • Promotes teamwork, collaboration, and cooperation among treatment teams to enhance patient and employee satisfaction, improve patient health outcomes, and maintain a safe and culturally sensitive care environment.
  • Manages routine and emergency physical health care needs by assessing, formulating and directing treatment interventions through collaboration with the treatment teams as needed.
  • Ensures provision of education regarding medications, relevant treatment information and disease process to patients and family members.
  • Works collaboratively in consultation with the program psychiatrists to institute actions enhancing the quality of physical medical services rendered to patients.
  • Utilizes evidenced-based care, best practices, and community standards to provide patient care.
  • Responsible for ensuring optimal provider productivity
  • Complies with eligibility and service provision requirements, treatment plan authorization and documentation, service provision documentation, and timely data submission.
  • Performs related work as assigned. Adapts to changing work requirements.
  • Demonstrates ethical conduct in all clinical and professional activities.
  • Complies with Agency policies and procedures, accrediting/regulatory bodies and performance contract documentation requirements.


What qualifications you will need:

Education:
  • Graduation from an approved college of medicine or osteopathy, with completion of Family Medicine, Internal Medicine or Pediatric residency training. Must be Board eligible (and preferably Board certified in or certified in Family Medicine, Internal Medicine or Pediatric Medicine.

Experience:
  • Prefer experience in ambulatory primary care, community health, and/or behavioral health.
  • Familiarity with the SMI population.
  • Ideal candidates must have 3 to 5 years of experience in a leadership role and supervisory experience.
  • Experience with a diverse and unserved population in an urban setting.


Other Key Qualifications
  • Must be licensed to practice medicine in the State of Texas according to the Texas Board of Medical Examiners.
  • Must have a DEA number, a Texas Department of Public Safety Controlled Substance Number, and Triplicate Prescriptions for duties related to patient care.

Knowledge, Skills and Abilities:
  • Extensive Knowledge of current healthcare needs and issues of children, adolescents, and adults
  • Must have knowledge of various therapeutic modalities and diagnostic evaluation.
  • Must have basic knowledge of principles of the development, implementation and documentation of individualized care and treatment.

  • Ability to relate and communicate well with all cultural and ethnic groups in the community, including fluency in written and spoken English. Bilingual skills in written and spoken Spanish are preferred.
