POSITION HIGHLIGHTS

The Program Operations Specialist is the primary point of contact and coordinator for the Bed Readiness Program. The Specialist serves as the keystone for patients and families/decision makers, the health care team including, physicians, advanced practice providers, RNs, rehabilitation specialists and social services as well as external agencies. The Specialist will lead coordination of post- acute care support to the SNF and assist the SNF in coordination of the patient's transition from The Bed Readiness site to the community. The Specialist may also be involved in process improvement initiatives and other special projects as assigned by Care Management Director and more.

PRIMARY JOB RESPONSIBILITIES:

• Work with HMC care teams to identify patients for discharge

• Coordinate with Social Work Department to ensure all critical information is effectively communicated to Skilled Nursing Facility

• Weekly on-site visits with staff for care coordination and conduct patient assessments

• Assist, resolve or appropriately refer customer service issues

• Maintain database for clinic, inpatient and BRP metrics

• Monitor length of stay on weekly basis

• Participate in patient care conferences and need and requested by staff

• Check re-admits through emergency department, are re-admitted to a hospital, or exceed the anticipated length of stay and required contract extension to make improvements to the process

REQUIRED POSITION QUALIFICATIONS:

A Master of Social Work Degree from a program accredited by the Council on Social Work Education AND 4-5 years of Social Work experience. Social Work supervisory experience is preferred.
