Pay Range
Pay Range: $ 18.89 - $ 25.51 Hourly

POSITION SUMMARY:
Provides comprehensive mental health and/or dual diagnosis services for individuals who have a diagnosable mental illness. The therapist functions as the primary clinician and is responsible for assessment, treatment planning, therapy/counseling, and education services for patients and families. Systematically and continually assesses the patient's mental condition in collaboration with the patient and family.
TYPICAL DUTIES:
- Develops, reviews and evaluates the needs for psychiatric case management services for a target population in collaboration with a program coordinator who is a mental health professional; acts as liaison to community providers and explains program goals to individuals and groups; screens referrals for psychiatric case management and makes recommendations to the Program Coordinator about assignments of Psychiatric Case Managers to patients.
- Supervises and supports a caseload of psychiatric patients in their acclimation to new community environments; social adjustment and interpersonal relationships; employment opportunities; avocational interests; understanding and use of public transportation; and knowledge of available recreational or leisure time activities.
- Independently carries out strategies for implementation of the care plan by establishing contact with various service providers and family members to: link patients with existing resources; assure appropriate utilization of life support services in the community; assure the smooth and orderly transition of patients into their respectively designated placement; take initiative for suggesting appropriate community services that would enable the patient to function at optimal level in the community.
- Responsible for maintaining contact with service providers to assess: appropriateness of placement; the continued progress of patients; and other supplemental needs of patients. Implements the proper intervention strategies with minimal supervision from the mental health professional.
- Responsible for documenting patient contacts and patient progress in the medical record and assists patients with paper work, correspondence, etc., necessary to ensure linkage to appropriate resources.
- Conducts training for new Psychiatric Case Managers and provides input to the Program Coordinator regarding the performance of Psychiatric Case Managers assigned to team; provides guidance and direction to Psychiatric Case Managers regarding role performance, community resources and program policies.
- Responsible for visiting patients in their community setting in order to assess their progress toward meeting the goals of the care plan and to monitor compliance with the treatment plan. Teaches, guides and assists patients and family members to insure their clear understanding of the patient's needs and expectations of the specific care plan and makes recommendations regarding modifications to treatment plans to the mental health professionals.
QUALIFICATIONS:
- Associate's degree in health-related field, 2-3 years' experience as a psychiatric case manager; or an equivalent combination of education and experience required.
- 2-3 years' experience as a psychiatric case manager; or an equivalent combination of education and experience required .
- Must possess a valid New York State Driver's License and access to automobile.
