Public Health Division Southwest Region Children's Medical Services offers medical case management for eligible children from 0-21 years old with special health care needs or chronic health conditions, to include those who may not be eligible for coverage elsewhere. Children's Medical Services also manages newborn genetics and hearing screening programs and hosts many specialty care outreach clinics.

Why does the job exist?

This position will provide direct medical social work services to clientele in the Southwest Region, in Grant, Luna, and Hidalgo Counties, and may include other counties as assigned and needed. Services and concerns are addressed on an individual, family level and/or on a systems level. This position will plan, implement and provide a safety net and assurance services for children and youth with special healthcare needs. All applicants for this position must be licensed in accordance with the Social Work Practice Act, Sections 61-31-1 through 61-31-25 NMSA 1978, and 16.63.1 through 16.63.20 NMAC.

How does it get done?

This position will be responsible for:
- Providing medical case management and coordination of service delivery and advocacy for children and youth with special healthcare needs.
- Coordinating and managing specialty outreach clinics within the SW Region as necessary by working collaboratively with UNM medical providers.
- Providing outreach and engages families, clients with community providers, programs and agencies.
- Provide a complete bio-psychosocial assessment and periodic reassessment regarding client¿s/family¿s needs.
- Work collaboratively with medical, educational, vocational and human service providers.
- Provide quality assurance for maximizing billing and on-going care coordination.
- Assure the cultural and linguistic needs of clients in the community are addressed and clients receive support in navigating a highly complex, medical system and can work across healthcare agencies and insurances.

Ideal Candidate

The ideal candidate has experience:
-Must possess and maintain a current social work license from the State of New Mexico.
- Strong preference will be given to bilingual applicants fluent in both English and Spanish.
- Providing social work practice services to clients and their families with medical conditions in medical and public health settings.
- Providing crisis intervention, formulating interventions and service plans.
- Conducting interviews for intake, assessment, diagnostics, and referrals.
- Working in partnership with multiple agencies and diverse client populations.

Minimum Qualification

Bachelor's degree in Social Work (in accordance with NMAC 61-31-9) and two (2) years of experience in a social work setting such as social service agencies, healthcare, medical, educational settings, mental health, home health, hospice, developmental disabilities and other relevant experience, working with patients, clients, providers and families. Employment Requirements

Must possess and maintain a valid Driver's License. Must possess and maintain a current Defensive Driving Course Certificate from the State of New Mexico as a condition of continued employment. Successful completion of Health Insurance Portability Accountability Act (HIPPA) and successful completion of Blood-borne pathogen training within thirty (30) days of employment.

Working Conditions

Services are provided in communities, clinics, auxiliary site settings, and in the homes of consumers to a diverse racial and ethnic population, many of whom do not speak English. Reaching and lifting up to fifty (50) pounds may be required. Potential exposure to communicable diseases. Travel to provide outreach and coverage, to attend meetings and training sessions, with the occasional overnight stay is required.
