Purpose of Position
The Maternal Child Health Nurse is a registered professional nurse who collaborates with the Family's healthcare team to best support positive outcomes for Mother and Baby. The Maternal Child Health Nurse along with the Public Health Team promotes prevention and educational services for persons of all ages throughout our service area, including communicable disease, emergency preparedness for a pandemic occurrence, immunization clinics, community health clinics and health education and screening programs

Description
1. Assures compliance with Nurse Practice Act and Standards of Practice in the State of Massachusetts.
2. Functions within the parameters of knowledge and experience, practicing safely and competently.
3. Works collaboratively as member of a team in providing services to families who are pregnant or have recently given birth.
4. Conducts visits at the family's home to assess needs and strengths related to overall health and well-being.
5. Engage, assess and present information in an unbiased, culturally conscious and appropriate manner, particularly related to sensitive topics.
6. Offer referrals for services to individuals with an identified need that may best suit the needs of the family.
7. Collaborate with referral sources and local healthcare entities in maintaining strong partnering relationships.
8. Provides health education including healthy lifestyles for women and children, childbirth education, parenting skills, care of infants and children
9. Promotes immunization and wellness programs through community outreach activities and patient visits.
10. Communicate with Physicians and counsel patient and family members as appropriate in relationship to communicable disease, wellness education and prevention guidelines.
11. Utilize results of monitoring and evaluation activities to identify education needs, improved patient care practices, and community outreach. Make training recommendations to HR Education Manager.
12. Consistently provides service excellence to all patients, family members, visitors, volunteers and co-workers.
13. Challenges current working practices; identifies process improvement opportunities and presents recommendations and solutions to management. Engages and commits to the organization's culture of continuous improvement by actively participating, supporting, and promoting CCHC Pillars of Excellence.
14. All other duties as assigned.

Qualifications
· Registered Nurse currently licensed in the State of Massachusetts.
· CPR required.
· Minimum of two (2) years clinical experience.
· Strong clinical and interpersonal skills
· Efficient computer skills
· Certified Lactation Counselor a plus
· Multi-lingual a plus
