Services include care management, practice support, annual wellness visits, health coaching, free clinic wrap around services, screenings/care gap closures and SDOH coordination, just to name a few.
Position Summary
Apply improvement methods to achieve successful outcomes: Use evidence-based structured improvement methods for all quality improvement projects. Promote the use of improvement methods and science throughout the organization.
Minimum Requirements
Bachelor degree- or higher- in healthcare/business related field
5 years experience in healthcare setting
Certification in specialty preferred in 3 years and recertified every 2 years
