Job Duties

Provides a range of casework management services, programming, and guidance to a geriatric Inmate population in a correctional facility. Plans, implements and coordinates reentry and case management services and serves as a liaison with community mental health, community corrections and other appropriate community resources. Ensures that follow up services for Inmates with special needs are available upon release.

Minimum Qualifications

Relevant professional experience (preferably under supervision) with progressively responsible experience in provision of social work services to varied population. Knowledge of the principles and techniques of social work Demonstrated knowledge and experience in aftercare and release planning for special needs populations. Knowledge of community resource access and utilization. Demonstrated ability to communicate effectively, orally and in writing and to work in collaboration with other institutional staff as well as outside agencies.

Additional Considerations

Advanced knowledge in social work, rehabilitative counseling or another related human services field.
