Full-Time Registered Dietitian

Location: Park Ave Extended Care Facility, Long Beach, NY

Facility Size: 240 beds

Park Ave Extended Care Facility is seeking a dedicated and experienced Registered Dietitian to join our team. If you are passionate about providing exceptional care and making a positive impact on residents' lives, we would love to hear from you.

Qualifications:

• Must be a Registered Dietitian with the Commission on Dietetic Registration.
• Experience in long-term care or short-term rehabilitation is a plus.
• Knowledge of EMR (SigmaCare) is a plus.
• Familiarity with Geri Menu is a plus.

Key Responsibilities:

• Conduct new and re-admission assessments.
• Perform quarterly, annual, and significant change assessments.
• Monitor and document residents' weights, weight loss, pressure ulcers, and wounds.
• Oversee residents undergoing hemodialysis and those on enteral feeding.
• Complete MDS (Minimum Data Set) documentation.
• Update care plans and participate in care plan meetings.
• Update meal and snack preferences, enter diets, and assist with menu reviews for residents.
• Conduct meal rounds and audits as assigned.
• Provide nutrition education and counseling to residents and their families.
• Attend morning reports as needed.
• Maintain strong verbal and written communication with the medical team, including MDs, NPs, nursing, and interdisciplinary teams.
• Perform related duties as required.
