Job Duties:
• Coordinate nutritional care of residents and patients by completing nutritional assessments
• Collaborates with IDCP team on care planning and nutritional recommendations
• Participate in QAPI and IDCP meetings as scheduled by facility
• Participate in monthly dietitian meetings
• Provide nutritional counseling and education for residents and family
• Completes comprehensive assessments on patients as required
• Conduct tray line audits as needed to assess for accuracy of ticketed items
• Complete Test trays and in services for dietary department
• Under PDPM, identify, document, and treat nutrition related diseases for improved outcomes
• Meet DOH/state/federal and company standards
• Perform duties independently without training or direction
• Perform other duties as assigned by management
• Assist director of dietary and provide kitchen oversight as needed
Required Skills/Abilities:
• Motivation and drive to complete caseload to drive positive outcomes
• Good decision making and judgment capabilities
• Ability to identify and solve complex problems
• Great communication and interpersonal skills are a must
• High comfort level with enteral nutrition, wounds, weight management, Hemodialysis
• Comfortable working in a healthcare facility and kitchens
• Familiarity with state survey process and compliance with nutrition care regulations
Education and Experience:
• Bachelor's degree in nutrition, health, or related field
• Must possess Registered Dietitian licensure within the state of operations
• At least one year of experience in a long-term care setting
• Experience with MDS 3.0
• Experience with electronic charting such as Point Click Care is a plus
Physical Requirements:
• Prolonged periods standing and walking
• Must be able to lift up to 25 pounds at time
