Case Management Registered Nurse (RN):
A Case Management Registered Nurse is a nurse who specializes in coordinating and managing patient care to ensure that patients receive the right care, at the right time, and in the right setting. They work in hospitals, insurance companies, and healthcare organizations, among other settings.
Responsibilities may include:
Assessing patients' health status and medical history
Developing and implementing care plans
Coordinating with interdisciplinary teams to ensure that patients receive appropriate care
Monitoring and evaluating patient progress
Assisting patients and families in navigating the healthcare system
Ensuring that patients receive appropriate care in a cost-effective manner
Documenting patient care and outcomes.
Requirements:
A current Registered Nurse (RN) license
A Bachelor of Science in Nursing (BSN) degree, although an Associate Degree in Nursing (ADN) may be sufficient in some cases
Strong communication and interpersonal skills
Ability to work independently and as part of a team
Knowledge of healthcare regulations, insurance requirements, and community resources
Strong organizational and time management skills.
This is a not full time but contracted travel position.
