Job Summary

Directly responsible for the day-to-day operations of the Right From the Start (RFTS) and Prevention First Projects on the regional level. Assumes responsibility for implementation, operation and management of the RFTS Project components, coordinating the care of eligible pregnant women and infants with many providers, professional specialists, and community resources.

Responsibilities

• Coordinate the health, education, psychosocial, and nutritional care for clients enrolled in the Right From the Start and Prevention First Projects. • Receive referrals from Designated Care Agencies within the RFTS Region and Medical Providers. • Complete assessment to identify barriers to a healthy outcome. • Develop service plan with client. • Arrange for interventions which meet identified needs. • Make home visits and contact client as needed. • Document client care. • Update service plan as needed. • Participate in interagency/interdisciplinary service plan meetings. • Advocate for the needs of the family; including: food, shelter, safety, crisis intervention, transportation and child care. • Maintain department, institution, and licensure (if applicable) required education. Document appropriately. • Demonstrate positive customer service and co-worker relations. • Participate in the continuous, quality improvement activities of the department and institution. • Perform work in a cost-effective manner. • Perform related duties as assigned. • Oversees and carries out all duties required for the implementation, operation, and management of the RFTS Project components. • Directly supervises the support staff and performance of the support staff's work. • Provides direction to and monitors agencies providing direct services to clients in accordance with project standards and protocols. • Plan departmental goals in alignment with CAMC Strategic goals and key critical processes. • Anticipate and update programs to meet changing practices within the healthcare system. • Collaborate with physicians to facilitate appropriate referrals, client care coordination and clinical quality. • Assists the Nurse Manager by performing Charge Nurse accountabilities in supervision and evaluation of performance of support staff. • Participate and plan departmental goals with the Nurse Manager, Medical Director, and Office of Maternal Child Health in alignment with CAMC Strategic goals and key critical processes. • Maintains Clinic Competencies in the role of Designated Care Coordinator Maternal Child Care Coordinator
