Job Summary and Qualifications

Seeking a Registered Professional Nurse who is responsible for assessing, planning, implementing, and evaluation the nursing care for patients suffering from chronic and end stage organ disease and potential transplant patients and live donors. The RN coordinates care delivered by other members of the transplant team to affect positive outcomes for the patient. If you are new to Transplant- we are willing to train an experienced RN that may be seeking a different career path in healthcare.

What you will do in this role:

Pre Transplant Phase

  • Assures the performance of necessary studies to determine a patient's candidacy
  • Responsible for the educational process for both patient and family
  • Participates in the evaluation and patient selection process
  • Monitors patient status throughout work-up and listing
  • Participates in the process for patient preparation for transplantation surgery
  • Responsible for data collection/entry/management for all regulatory agency requirements

Transplant/Inpatient

  • Assumes lead in directing responsibility of the education to patient and family
  • Participates in clinical rounding that includes monitoring quality and adverse changes
  • Maintains communication with all the multi-disciplinary team
  • Acts as the resource person regarding transplantation for all staff
  • Monitors ongoing plan of care and implements changes accordingly
  • Prepares patients for discharge and outpatient follow-up
  • Communicates with referring physicians as indicated for continuity of care
  • Responsible for communication from inpatient to outpatient clinical continuity of care
  • Responsible for continuing data collection/entry/management for all regulatory agency requirements

Post Transplant Phase

  • Monitor and follow all diagnostic studies
  • Evaluate health status as per clinical protocol
  • Communicate and coordinate any patient issues/concerns to the appropriate transplant team member
  • Continues educational process to patient and family members
  • Provides patient care under the direction of the transplant physician
  • Responsible for data collection/entry/management for all regulatory agency requirements

What qualification you will need:

  • Graduate of a nursing program: diploma, associate degree, or baccalaureate program.
  • At least 1 year RN experience and must meet ALL requirements of RN II
  • Basic Cardiac Life Support must be obtained within 30 days of employment start date

Employee completes initial and ongoing training and competencies as defined by service line, facility and department/unit which are specific to the needs of the patient population served (if applicable).

Minimum License, Certificates, or Registrations Required:

  • Valid and Current Texas License is required - Registered Nurse
