Description

Responsibilities:

Follow a designated care plan in accordance with patient's needs

Make home visits to clients in designated geographic territories

Perform assigned duties, including administration of medication, wound care, treatments, and procedures

Monitor clients' conditions, reporting changes to Clinical or Client Services Manager

Follow up with, execute, and properly document doctors' orders

Perform client assessments as necessary

Case management and coordination

Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.

Demonstrate caring and empathy

Take and track vital signs

Measure and administer medications as prescribed

Provide wound care, as needed

Provide catheter care

Ensure patient is fed and bathed

Ensure bedding is clean

Track and order supplies

Willingness to work a variety of shifts

Reliable transportation to the residential setting

Must feel confident providing nursing care without oversight

Ability to create rapport with patient and family members

Excellent communication skills

Education:

Associate's degree in nursing, required

Bachelor's degree in nursing, preferred

3+ years of nursing experience in a residential setting

Additional details
ALL APPLICANTS MUST HAVE 1 year Home Care experience. 1 year Case Management Experience. Ability to travel to cases as assigned. Weekend rotations are once every 6 weeks. Job Summary: As a Home Health RN, responsibilities not limited to assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care is delivered within organizational philosophy, policy and standards of community health practice. Also required to perform admission processes and case management responsibilities in accordance with MGB Home Care’s policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.
