Job Description:

The Registered Nurse (RN) in Home Health provides compassionate and comprehensive nursing care to patients in their residential settings. The RN follows designated care plans tailored to individual patient needs and collaborates closely with healthcare teams to ensure optimal patient outcomes.

Responsibilities:
  • Follow a designated care plan according to the patient's needs.
  • Conduct home visits to clients in assigned geographic areas.
  • Perform nursing duties, including administration of medication, wound care, treatments, and procedures.
  • Monitor clients' conditions and report any changes to the Clinical or Client Services Manager.
  • Follow up on and document doctors' orders accurately.
  • Conduct client assessments as required.
  • Manage case coordination and ensure continuity of care.
  • Document observations, interventions, and evaluations related to client care using a state-of-the-art touch pad tablet.
  • Exhibit caring and empathetic behavior toward patients and their families.
  • Measure and administer medications as prescribed.
  • Provide wound care and catheter care as needed.
  • Assist patients with personal care, including feeding and bathing.
  • Ensure cleanliness of bedding and overall patient environment.
  • Track and order medical supplies as necessary.
  • Be willing to work various shifts as required.
  • Utilize reliable transportation to reach residential settings.
  • Provide nursing care confidently and independently.
  • Establish rapport with patients and family members.
  • Maintain excellent communication skills with patients, families, and healthcare teams.

Qualifications:
  • Education:
    • Associate's degree in nursing (required)
    • Bachelor's degree in nursing (preferred)
  • Experience:
    • Minimum of 3 years nursing experience in a residential or home health setting.

License/Certification:
  • Current RN license in the state of practice.
  • BLS certification preferred.

Skills:
  • Strong clinical assessment and critical thinking skills.
  • Ability to work independently and make decisions in a home care environment.
  • Excellent communication and interpersonal skills.
