Registered Nurse (RN) Home Health /Sign on Bonus

We are seeking to hire an experienced RN to join our ever-growing interdisciplinary team of home health professionals.

RN Case Management Expectations:

• Provides direct patient care in their home.
• Oversees the patients' plan of care.
• Coordinates the patients' ongoing skilled care under the direction of patients' physician.
• Promotes positive patient outcomes.
• Participates in patient interdisciplinary case conference.

Requirements to join the team as a RN :

• Registered Nurse license required
• Ideal candidates will have home care experience but we are open to training RNs with a ICU/Med-Surg/Emergency Room background
• Valid driver's license and auto insurance
