REGISTERED NURSE (RN CASE MANAGER) - Position Summary

The Registered Nurse (RN Case Manager) plans, organizes and directs hospice care. Experienced in nursing, with emphasis on community health education/experience. The Registered Nurse (RN Case Manager) builds from the resources of the community to plan and direct services to meet the needs of the individual and families within their homes and communities.

REGISTERED NURSE (RN CASE MANAGER) - Essential Duties & Responsibilities

Patient Assessment and Care Planning
  • Completes initial, comprehensive, and ongoing assessments of patients and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).
  • Uses health assessment data to determine nursing diagnosis.
  • Assesses and evaluates patient's status by writing and initiating a plan of care, regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care if necessary.
  • Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the physician in the physician's plan of care.

Clinical Care and Nursing Interventions
  • Provides professional nursing care by utilizing all elements of the process.
  • Initiates appropriate preventative and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician's plan of care and/or orders.
  • Assists the patient with the activities of daily living and facilitates the patient's efforts toward self-sufficiency and optimal comfort care as needed.
  • Provides and maintains a safe environment for the patient.

Patient and Family Education and Support
  • Counsels the patient and family in meeting nursing and related needs.
  • Provides health care instructions to the patient as appropriate per assessment plan.
  • Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet, and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.
  • Works in cooperation with the family/caregiver and hospice Interdisciplinary Group Members to meet the emotional needs of the patient and family/caregiver.

Documentation and Communication
  • Completes, maintains, and submits accurate and relevant clinical documentation regarding the patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate in the EMR at bedside.
  • Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; obtains/receives and documents physicians' orders as required.
  • Coordinates arrangements for equipment and other necessary items and services are available.

Interdisciplinary Collaboration and Teamwork
  • Works in concert with the interdisciplinary group.
  • Attends interdisciplinary group meetings.


REGISTERED NURSE (RN CASE MANAGER) - Qualifications/Experience
  • Bachelor of Science in Social Work. MSW preferred.
  • Minimum of one year's experience in health care, hospice experience preferred. Understands hospice philosophy, and issues of death/dying.
  • Experience in hospice care preferred.
  • Demonstrates good verbal and written communication, and organizational skills.
