DESCRIPTION

The Infectious Disease (ID) Clinic brings together specialists who treat infections in surgical, orthopedic, oncology, transplant and foreign travel patient populations. This position will primarily provide nursing care across 6 branches of the Infectious Disease Clinic including COVID Clinic, Transplant, Oncology, Orthopedic, General ID and Outpatient Parenteral Antibiotic Therapy (OPAT) program.

The Clinical Telephone Triage Nurse will provide nursing and administrative support to the physicians who provide care in the clinical setting. This position is responsible for: provide advanced clinical telephone support to identify concerns and issues of caller, prioritize calls and provide assessment using their clinical judgment in addition to following approved protocols to provide care, advice, disposition and/or resolution to caller's concerns, will perform follow up calls and other clinical services over the phone or by electronic means (such as in-basket messaging) per protocol, will be responsible for care coordination, and will act as a liaison between patients, pharmacies, home care companies, skilled nursing facilities and the ID providers.

The candidate must possess strong critical thinking, telehealth and communication skills to collaborate and coordinate care with the ID health care team. The candidate will be a vital part of the ID team to ensure optimal care for patients and families who are dealing with complex health issues.

MINIMUM QUALIFICATIONS
  • Must be a graduate from an accredited school of nursing.
  • Registered Nurse (RN) license issued by the state of California.
  • BART or BLS at time of hire with commitment to get BART within six (6) months of hire date.
  • Three or more (3+) years of Infectious Disease RN experience (OPAT, COVID General ID).
  • Three years of RN experience in an ambulatory clinic.
  • Three (3) years of telephone triage experience with strong critical thinking skills.
  • Current experience in EPIC/EMR experience.
  • Experience working with adults who might require additional social and medical support.
  • Excellent interpersonal, organizational, planning and leadership skills.
  • Must be able to function in a fast-paced environment, have excellent interpersonal and communication skills, and consistently demonstrate empathy and kindness to all staff and patients.
  • Strong critical thinking and problem-solving skills to manage multiple levels of information and responsibilities, and to quickly assess moderately-complex problems to develop potential solutions, use of evidence-based clinical practice is essential.
  • General knowledge of payer industry, resource management, and reimbursement,
  • Proficiency in Information Systems and computer programs including MS Office (Word, Excel Outlook, etc.).
PREFERRED QUALIFICATIONS
  • BSN.
  • Home health and/or hospice experience.
  • Experience with referral management
  • Prior case management experience.
  • Utilization review, discharge planning, and managed care experience.
  • Home Infusion experience.
  • Typing and data entry skills.

  • Must be able to work various hours and locations based on business needs.
  • Employment is subject to a criminal background check and pre-employment physical.
  • Selected candidate may be hired at the CNIII/CNIV level pending review and approval by the Nurse Executive Council.
Pay Transparency Act

Annual Full Pay Range: $123,902 - $173,659 (will be prorated if the appointment percentage is less than 100%)

Hourly Equivalent: $59.34 - $83.17

Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable).
