Responsibilities

  • We are searching for a devoted RN case manager to coordinate and advocate for our patients' care & create an appropriate discharge plan.
  • The RN case manager will be required to review patients' charts, and to meet

    with families and healthcare providers to establish the most suitable course of action.
  • The Case Manager should remain in contact with patients and their families during the hospitalization stay.

Qualifications

Job Requirements:
  • Must maintain current Texas RN License
  • BLS Required.
  • 2 years prior clinical experience, preferably in inpatient Behavioral Health, or home health case-management environment
  • ADN required
