Prior case management experience preferred
Monday thru Friday 8am - 5pm with some weekend and holiday coverage

- Complete referral process.
- Explain home health and/or hospice services and facilitate continuity of care.
- Assist with communication or payer sources. Determine benefit eligibility.
- Obtain, record and disseminate referral information.
- Document clinical records to ensure compliance with regulatory standards, agency policies and procedures.
Licensure / Certification / Registration:
- Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date required.
Education:
- Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.
