  • The Team Coordinator is a registered nurse (RN), who works in the best interest of Baylor Scott & White Surgical Hospital Fort Worth (BSWSHFW) and accepts additional responsibility to support the Mission, Vision and Values of BSWSHFW.
  • The Team Coordinator is responsible for the smooth and efficient patient flow within the department and throughout the facility.
  • Delegates assignments and provides supervision for the support staff in accordance with their level of training and the patient's acuity.
  • Works, in cooperation with all members of the health care team to ensure the quality of patient care.

Required Skills:

Education

• Graduate of an accredited school of nursing, BSN preferred

Licensure/Certification

• Current Texas RN License

• Current BLS, must get ACLS within 6 months of hire

Required Experience:

Specific Job Experience
  • Minimum two (2) years' experience as an RN in a hospital Medical/Surgery or ICU Unit


Knowledge, Skills and Abilities

• Demonstrates knowledge and skill level necessary to assess patient's psychosocial, educational and physical needs, as well as the ability to address these needs in an age appropriate manner in the pediatric, adolescent, adult and/ or geriatric patient population.

• Excellent communication skills, verbal and written.

• Good organization and time management skills.

• Ability to make decisions based on nursing judgment.

• Able to prioritize and organize workload to meet needs of patients and unit

• Experience with Electronic Healthcare Records

Principal Duties and Responsibilities:

• Demonstrates clinical competency, knowledge, and possesses sound critical thinking and clinical judgment

• Monitors daily performance of clinical team members to ensure duties are completed accurately, efficiently, and timely

• Models standards of care and assists in direct patient care as needed. Demonstrates respect, equity and empathy in actions and interactions with patients, providers and team members.

• Assigns patient care to team members based of levels of experience, competency and knowledge to enable optimal patient outcomes.

• Communicates professionally; demonstrates the ability to use strategies and available resources for problem-solving for patients, family, and staff

• Uses chain of command when appropriate for further intervention.

• Evaluates performance of clinical team members, assists in monitoring behaviors and competencies

• Performs leader rounding with patients and families to ensure that needs are met and identifies opportunities for improvement.

• Collaborates with clinical leaders on continuous quality improvements. Promotes and participates in facilitating a shared governance model.

Prioritizes work and resources to coordinate patient care delivery in a financially responsible manner.
