Resident Care Coordinator- RN or LPN

Canterbury Place, a premier senior living community in Pittsburg, is seeking a compassionate and dedicated Resident Care Coordinator (RCC) to join our team! As an RCC, you will play a vital role in ensuring the well-being and happiness of our residents in our Personal Care Homes (PCH).

Responsibilities:

• Oversee the daily care needs of assigned residents, working closely with caregivers and other healthcare professionals
• Develop and implement individualized care plans that promote resident well-being and independence
• Monitor residents' health status and report any changes to nurses
• Assist residents with activities of daily living (ADLs) such as dressing, bathing, and medication management
• Ensure a safe and secure environment for residents
• Staff scheduling, staff oversight, filling in for shifts (if needed), assessing potential residents, etc
• Respond to resident concerns and requests promptly and compassionately
• Maintain accurate and complete resident records
• Supervise Med Techs/NA's
• Flexibility to be able to work the floor/pass meds/assist with care & documentation as needed
• Collaborate with families to keep them informed and involved in their loved one's care

Qualifications:

• Minimum of 2 years of experience working with seniors in a long-term care or assisted living setting
• RN or LPN license in good standing in the State of Pennsylvania
• Strong understanding of senior care best practices
• Excellent communication, interpersonal, and organizational skills
• Compassionate and patient demeanor
• Ability to work effectively as part of a team
• Positive and enthusiastic attitude
