Job Title: Resident Services Coordinator

Job Summary

Assist residents in identifying appropriate resources in order to maintain their highest level of independence and satisfaction to include problem solving for health, social, and emotional needs and coordinating support services.

Essential Job Functions

  • Match residents to service providers in the community. Including case management, counseling, transportation, food delivery.
  • Coordinates new resident orientation processes to ensure a smooth transition and appropriate socialization activities.
  • Communicates with health care providers regarding health conditions, safety issues, and the needs of all residents.
  • Facilitate family conferences to determine the amount of support services required to meet the resident's needs.
  • Educate residents on availability of services, application procedures and provide advocacy as needed.
  • Coordinate programs and wellness events
  • Work with Program Manager Recreational Activities to maintain monthly newsletter and calendar
  • Assist in training residents on the obligation of tenancy
  • Respond to management requests regarding resident quality of life projects and special assignments
  • Complete all necessary and required documentation to include psychosocial assessments, supplemental notes, quarterly evaluations, etc
  • Provide coverage for front desk as needed
  • Attend meetings as needed


Job Requirements

Education:

  • Bachelor's Degree preferred or equivalent experience


Experience:

  • One to two years of experience working with elderly residents, and previous experience with identifying community resources in a geriatric setting, preferred.


Licenses/Certifications:

  • CPR Certification preferred


Abilities Required

  • Pleasant personality, interpersonal skills necessary to interact with all levels of the work group residents, families, and volunteers.
  • Spanish and/or French preferred
