Overview
Establishes and maintains a welcoming, safe, therapeutic and family friendly environment in Molly's Place while also providing assistance to patients, families, and health care providers. Identifies both clinical and community resources related to social determinants of health and actively refers patients and families to the support services needed. Relays relevant information to providers so that they can best serve CHaD families.

Responsibilities
  • Creates and maintains a therapeutic and safe environment which supports child development, family systems, and diversity.
  • Advocates in conjunction with patients and families in obtaining needed services and/or resources for which they are eligible.
  • Work closely with the health care team and community partners to help ensure that patients and their families have comprehensive and coordinated care.
  • Approaches all interactions with cultural competence and a supportive world view
  • Identifies and contributes to the development of systems and resources needed to support the needs of patients and families as well as the staff and institution.
  • Facilitate use of resources available within Molly's place, develop sustainable programming for new resource programs, provide education to patients, families, and staff about all resources available. Maintain a commitment to resource and referral quality improvement.
  • Develop and maintain family center resources, including but not limited to the lending library; family food and coffee program; resource guide; and fuel voucher program.
  • Track family center data
  • Works with patient, family, and medical team to assess patient eligibility for financial assistance, government program inclusive of Medicaid, community agencies,
  • Documents services provided in the patient's medical record and applicable databases.
  • Understand how their role fits within the CHaD CORE and how to support families in navigating the team process.
  • Provide donor stewardship
  • Supervise volunteers and interns
  • Preforms other duties as required or assigned.

  • Qualifications
    • Associates degree in human services, child development, or family systems with a minimum of two years of human services experience. Bachelor's degree preferred.
    • Excellent interpersonal, communication, and organizational skills required
    • Strong emotional intelligence essential.
