JOB SUMMARY
  • The Respiratory Disease Navigator (RDN) is responsible for working directly with patients who are diagnosed with Chronic Respiratory Diseases such as Chronic Obstructive Pulmonary Disease/Emphysema, Asthma, Pulmonary Fibrosis and other chronic pulmonary diseases. The primary objective of the RDN is to help the patient become compliant with management of their chronic respiratory disease which in turn will improve the patients quality of life and reduce the risk of a readmission in less than 90 days. The RDN works in collaboration with patients, nurses, physicians and Therapists to ensure the patients have a thorough knowledge of their disease, purpose and use of medications, available support services (Examples: Pulmonary Rehab, Smoking Cessation, Support Groups, Home Health, etc.) and to ensure the patients have what they need in their home after discharge. The RDN will provide post discharge support to patients via phone calls and follow a systematic approach to visiting this patient population Monday through Friday. The RDN may be asked to perform RRT duties as needed.

  • Maintains current knowledge in present areas of responsibility (i.e., self education, attends ongoing educational programs)
  • Attends staff meetings and completes mandatory in-services and requirements and competency evaluations on time.
  • Demonstrates competency at all levels in providing care to all patients based on age, sex, weight, and demonstrated needs. For non-clinical areas, has attended training and demonstrates usage of age- specific customer service skills.
  • Wears protective clothing and equipment as appropriate.
WORKING CONDITIONS
  • General environment: Works in a well-lighted, air-conditioned area, with moderate noise levels.
  • May be exposed to limited hazardous substances or body fluids, or infectious organisms.
  • May be required to change from one task to another of different nature without loss of efficiency or composure.
  • Periods of high stress and fluctuating workloads may occur.
  • May be scheduled as needed including overtime


Qualifications

EDUCATION REQUIREMENTS
  • 2 year / Associate Degree in Respiratory Therapy (Required)
  • 4 year / Bachelor's Degree in Respiratory Therapy (Preferred)
EXPERIENCE REQUIREMENTS
  • 2 - 3 years Respiratory Therapy (Required)
  • 1 - 2 years computer system concepts and willing to learn new computer applications as needed (Required)
CERTIFICATIONS AND LICENSURES
  • Required Certifications/Licensures: Registered Respiratory Therapist (RRT); AHA BLS, ACLS, PALS
  • Must obtain a Specialty Credential in COPD and/or Asthma within 2 years
