Job Description
Collaborates with practice clinical leadership and Health Information Management to ensure the transition of clinical documentation within the electronic medical record. Ensures the accuracy and completeness of clinical conditions are captured within the new electronic health record to assist with reporting outcomes.
Job Responsibility
1.Collects and collates clinical information from patient electronic medical records, then transfer information accurately into the enterprise electronic health record to ensure appropriate level care is rendered to patients.
2.Makes decisions regarding clinical conditions that must transferred to mitigate any potential loss of productivity during the onboarding or transition to the new electronic health record.
3.Demonstrates knowledge of regulatory reporting requirements, medical documentation requirements, and organizational policies and procedures utilizing independent judgement.
4.Communicates and interacts with clinical staff, including Physicians and ACPs, to ensure accuracy and reconciliation of redundant clinical conditions are addressed.
5.Identifies potential clinical issues that require follow-up and resolution prior to patient encounters.
6.Review and process patient reported errors or requests for amendment of clinical documentation and assists medical staff with responding.
7.Ensure that queries from medical staff regarding clinical documentation within the electronic health record are reviewed and addressed.
Preferred Skills
Job Qualification
Bachelor's Degree required, or equivalent combination of education and related experience.
1-3 years of relevant experience required.
Licensed Practical Nurse in New York State, required.
