Cuero Regional Hospital is searching for a PRN RN Case Manager ER in our Case Management Department.

Job Description:
  • Under the Direction of the Chief Quality Officer, The Case Manager will ensure that patients receive appropriate, quality care that is cost effective.
  • Work directly with patients and their family members to conduct various assessments to determine the patient's needs.
  • Once the necessary data has been collected, the Case Manager works closely with the members of the patient's healthcare team to manage and coordinate all areas of the patient's care and any post discharge needs.
  • The duties of case manager nurses include monitoring and assessing the needs of patients, determining treatment eligibility, using clinical options to decide if plans meet the patient's needs, and making arrangements for additional services that help patient's achieve a positive outcome.
Qualifications:
  • Associate Degree in Nursing
  • Bachelors Degree in Nursing preferred
  • 1 year of hospital experience
  • 1 year of hospital case management & Interquel or other programs preferred
  • State of Texas Registered Nurse
  • BLS required
