RN Case Manager

Always Home Nursing Services, Inc., is hiring a Registered Nurse (RN) Case Manager for the Sacramento area.

Responsibilities

• Utilizes the nursing process to monitor, plan, implement and evaluate patient care

• Perform nursing assessments of clients in their home

• Supervise and teach nurses as needed

• Provide complete documentation for reports

• The Registered Nurse (RN) will perform duties within their scope of practice to meet direct patient care needs.

Qualifications

• Current California Registered Nurse (RN) License

• Current BLS card

• One-year prior Registered Nurse (RN) experience required
