JOB SUMMARY:

RN will work with residents, their family and the facility staff to coordinate care received by all disciplines throughout the course of their stay. The Coordinator will complete the MDS utilizing current RAI manual guidelines and oversee accurate and timely completion of all Medicare residents' MDS assessments. The coordinator will act as resource for the Medicare MDS Case Manager RN's, LTC nurses and all others requiring MDS support. The MDS coordinator will work in collaboration with the rehab director and nurse manager in an effort to provide excellence in the quality of care and customer service delivered by Marwood Nursing & Rehab.

JOB SPECIFIC TASKS & ESSENTIAL DUTIES:

  • MDS Case Managers will coordinate care for their assigned resident using a holistic approach.
  • They will collaborate with the entire interdisciplinary team and act as liaison to families, physicians and home care agencies as needed.
  • They will evaluate all aspects of care and be the primary team member with an all inclusive head to toe knowledge of the resident's medical condition and educational need.
  • They will act as a resource to the floor staff i.e. nurses, nurse aides, therapists, social workers and dieticians to provide education, role model and assist in care delivery as needed.
  • The will play an active role in the discharge process and will coordinate the discharge intent notice with the variety of disciplines.
  • They will assess the medical needs of the resident prior to discharge to make sure all educational opportunities have been addressed.
  • They will attend resident care plan meetings representative of nursing.
  • They will meet with the nurse manager, rehab director, social workers and dieticians daily for a brief update so there is a steady flow of communication amongst all team members.
  • Performs other duties as assigned.

QUALIFICATIONS:

Graduatefrom an accredited school of nursing.

Mustbe currently licensed to practice as a Registered Nurse and hold a valid Michigan State License.

Satisfactoryperformance in staff or charge nurse roles for a minimum of 2 years.

BeMDS 3.0 certified (or equivalent)

Mustdemonstrate competence in assessment skills and computer literacy.

Possessgood organizational ability

Possessgood oral and written communication skills
