A Brief Overview

The Diabetes Quality Coordinator (QC) is responsible for overseeing the day-to-day operations of the DSMES service at all sites. Provides assessment, planning, implementation, and evaluation of Diabetes Education Programs for patients and clinical professionals. The QC ensures that the National Standards (NSDSMES) are met and maintained at all times.
What You Will Do
  • Oversees the planning, implementation, and evaluation of the Diabetes Education Program
  • Monitors and facilitates maintenance of the Diabetes Program team members' qualifications (CE credits, training, competency, licensures and registration).
  • Responsible for maintaining ADA Recognition and participating in the evaluation of the Diabetes Program services's effectiveness.
  • Assesses and develops plan of care on an individual basis.
  • Provides diabetes education to patients and staff in the areas of diabetes and its management.
  • Provides inpatient diabetes education when outpatient schedule allows
  • Travel to the hospital for inpatient diabetes education

  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.


Qualifications:
Education
  • (BSN) Bachelor's Degree in Nursing (Required)
Work Experience
  • 3+ years RN experience. (Required) and
  • Diabetes Educator Experience (Preferred)
Knowledge, Skills, & Abilities
  • Academic courses and/or experience in program management and in the care of people with a chronic disease. (Required proficiency)
  • Certified Diabetes Educator and/or Certified Insulin Pump Trainer. (Preferred proficiency)
  • Skill and ability to communicate effectively both verbally and in-writing. (Required proficiency)
  • Skill and ability to handle multiple priorities and deadlines. (Required proficiency)
  • Possesses an ability to communicate with patients, families, staff, physicians, support agencies, vendors, and others (Required proficiency)
  • Has an ability to evaluate and/or interpret information and make independent decisions. (Required proficiency)
  • Ability to work as a member of a team. (Required proficiency)
  • Skill and ability in Microsoft Office and Zoom applications. (Required proficiency)
Licenses and Certifications
  • Registered Nurse (RN), Ohio and/or Multi State Compact License Upon Hire (Required) and
  • Basic Life Support (BLS) Upon Hire (Required)
  • Diabetes Care and Education Specialist within 2 Years (Required)
