Job Description:

A.
1. Assesses the needs of patients, and is instrumental in developing and/or contributing to the plan of care to facilitate the care of the patient with diabetes from admission through discharge.
2. Ensures plans are age-specific and meet the National Standards for Diabetes Education.
3. Participates in follow up appointments and/or phone calls for patients who attended the outpatient diabetes education program in accordance with the American Diabetes Association Accreditation.
B. Maintains accurate records and statistics toward efficient, effective, quality work practices and patient outcomes.
  • Demonstrates appropriate skill in utilization of the computer for automated order entry, data retrieval, and interdepartmental communication.
  • Demonstrates initiative and self-direction in completion of assigned responsibilities in accordance with department needs.
  • Contributes toward continuous quality improvement monitors, ie follow up phone calls/ statistics.
  • Assists with diabetes support groups.
C 1. Communicates with patients, families, physicians and other health care team members in achieving excellence in diabetes care and education.
2. Serves as a resource and provides ongoing access to patients/families toward adaptation to disease process and management skills.
3. Uses effective interpersonal communication skills toward collaborative practice and team building.
4. Collaborates with physicians and nursing, case management, and social work to facilitate discharge of inpatient with diabetes.

D 1. Provides bedside glucose monitor training for staff.

2. Provides education for use of bedside glucose monitoring system for newly hired nursing staff.

3. Certifies newly hired nursing staff for use of bedside glucose monitor equipment through direct observation and written testing.

4. Collates and disseminates appropriate paperwork to complete the certification process.

5. Actively participates in and supports departmental and organizational quality initiatives and goals.

E. 1. Identifies professional goals, which reflect own strengths and weaknesses and seeks assistance for improvement of performance.

2. Attends and participates in meetings according to department standards.

Job Qualifications:

QUALIFICATIONS :

Education:
1. Current RN licensure by the State Board of Nursing, Commonwealth of Pennsylvania.
2. Graduate of an accredited School of Nursing, BSN preferred.
3. Certified Diabetes Care and Education Specialist.
Experience:
1. Three to four years clinical experience
2. Teaching experience, both formal classroom and informal.

Other Skills:
1. Excellent interpersonal skills
2. Certified Accuchek Inform Operator, Accuchek Inform trainer preferred.
