Grace Heights Health and Rehabilitation is currently offering a $10,000 Sign On Bonus for a full-time RN MDS Coordinator to join our team of healthcare professionals in our 120 bed facility. We encourage a fun, family-like environment with a strong focus on resident care and we are looking for a motivated nurse to join our administrative team.

  • Must have current RN license
  • 2 years of LTC clinical experience
  • Experience with Medicare/Medicaid reimbursement, MDS completion, clinical resource utilization, and case management is desired
