Your Role:

Reporting to our Director of Nursing, you will play a vital role in enhancing the quality of life for our clients as you supervise care and develop a team of Care Professionals.

What You Will Do:

Supervise a team of Care Professionals in their delivery of care

Provide direct care on an as-needed basis

Provide scheduled on-call care supervision

Perform care scheduling on an as-needed basis

Lead team trainings

Develop client assessments and participate in admission and discharge process

Develop care plans

Perform care quality visits

Perform nursing care after receipt of our nursing agency license

Assist in the development and implementation of innovative care practices and procedures

Serve as back-up to our Director of Nursing when she is unavailable

Who You Are:

A compassionate and empathetic individual with a genuine desire to help others through;

Excellent communication, time management, team building and interpersonal skills.

Ability to multi-task and deal effectively in stressful situations.

Reliable, patient and adaptable in various situations. 

A positive solution-based attitude.

Hold an active Illinois registered nursing license.

2-3 years' experience as a clinical manager or nurse supervisor with a home health or home nursing agency.

3-5 years of progressive supervisory experience.

5+ years of home health or home nursing experience preferred.

Able to pass a drug test and background check and have reliable transportation. 

Proficient with WellSky software a plus.

Compensation and Benefits:  

• Base Salary: Between $80,000 and $90,000

• Incentive Bonus of Up to 10% of Base Salary Paid Quarterly
