Summary:

The Renal Care Coordinator, RN will be responsible for planning, implementing, and coordinating services and activities to improve care of the renal patient at Yuma Regional Medical Center. This individual is responsible for establishing and monitoring clinical performance criteria, assuring compliance with regulatory requirements. The Coordinator works with the Nephrologist/Medical Director to develop policies, procedures, enhance the electronic medical record, and maintain current education for patients, staff, and the Yuma community.

Responsibilities:

Coordinates the Renal Care Program at Yuma Regional Medical Center working closely with others and participating in interdisciplinary team meetings.

Maintains skills necessary to perform required surveillance; demonstrates in-depth understanding of infection prevention and control, reducing risk of bloodborne pathogen transmission.

Partners with Bio-Medical Services to assure patient care equipment remains in working order, safe for patient use, per manufacturer instructions and routine maintenance and disinfection.

Leads, participates, and collaborates in the development and implementation of evidence-based standards of care.

Collects, monitors and analyses on-going data regarding outcomes of the Renal Care Program, and presents required data to the appropriate Quality Council.

Develops an effective plan to address opportunities for performance improvement related to the care of renal patients.

Participates in performance improvement activities, providing feedback to staff.

Maintains responsibility for providing coverage to the department.

Other duties as assigned.

Other Information: Current AZ RN License, BLS, ACLS.

Bachelor's degree in nursing, 3+ years of nursing experience.
