As a Team Lead Recovery Care Coordinator (RCC) you will be at the forefront of Contessa's new and highly innovative care model that allows patients who are clinically the option to receive acute level care in the comfort of their own home in lieu of an inpatient setting. As a Team Lead Recovery Care Coordinator, you will leverage Contessa's proprietary workflow platform, advanced telehealth technology, and our interdisciplinary network of doctors, nurses, social workers, and ancillary providers to improve the health and well-being of our patients during what is often one of the most challenging times in the lives of our patients and their families.

The Team Lead Recovery Care Coordinator is the patient and RCC advocate and will lead patient support across all markets in multiple states throughout the care model. You will work directly with the patient's providers in the development of the patient's personalized care plan and be the primary point of contact regarding the patient's experience of receiving coordinated care in the home. You will be a backup for the Market Manager to lead team decision making and provide expertise and guidance in managing patient care concerns.

Responsibilities

  1. Fulfills the role of an RCC completing patient admissions
  2. Serves as a resource to all other RCC's in completing admissions
  3. Leads communication and coordination of care between all members of the care team.
  4. Assists all patients and care team members throughout the care model by acting as a patient advocate.
  5. Assists Market Manager in coordination of staffing and ensures appropriate resources are available to support patient care
  6. Ensures that all new RCCs have the appropriate tools and templates to operate.
  7. Supports Market Manager in coordination of clinical and operational workflows and documents those steps.
  8. Assists Market Manager in maintaining data and reporting accuracy
  9. Serves as a subject matter expert in identifying and solving for negative trends and hurdles involved in health care system and incorporates solutions for system challenges.
  10. Builds and maintains collaborative professional working relationships with all care team members and is available to provide guidance and support during times of urgency or critical needs.
  11. Assists as needed in recruiting, interviewing, and training of new team members.

Qualifications

Required
  • Education: ASN - Associate of Science in Nursing
  • Licensure: Current, active RN licensure in good standing and willing to be licensed in multiple states at Contessa's expense
  • Experience: 3-5 years of acute nursing experience and 2-3 years of RCC experience preferred. Possess a general knowledge of clinical standards and outcome measurement
  • Transportation: Must have reliable transportation and proof of liability insurance
  • Work Schedule: Ability to support after hours schedules and on-call needs.
  • Health: Able to pass required drug screen and communicable disease screenings
  • Background: Able to pass detailed criminal background check according to company policy

Preferred
  • Education: BSN preferred
  • Certification: Care Coordination and Transition Management (C.C.C.T.M)
  • Regional knowledge: Prefer knowledge of the geographical area of the assigned market
