Hanover Hall is seeking Social Services Coordinator to join our skilled nursing home in Hanover, PA .

The Social Services Coordinator is responsible for facilitating communication between the residents and the Facility's Care Team, family members and community partners. The Social Services Coordinator will assist in planning, organizing, implementing, evaluating, and directing the overall operation of the Social Services Department.

This is a F ull-Time , hourly , non-exempt p osition reporting to the Social Services Director . Compensation commensurate with education and experience.

Pay Rate: $18 - $23 /hr

Job Functions

  • Complete MDS reports, social histories, resident interviews and update care plans as needed.
  • Attend care plan meetings and other meetings as required.
  • Ability to listen and act compassionately with residents/family members; must be able to work on own and maintain professionalism.
  • Document residents' behavioral and psychosocial status
  • Provide assistance to the family and residents at all times of adjustment, crisis, or particular need .
  • Orients resident and family to Nursing center, helping both adjust to new environment .

Qualifications:

  • Bachelor's or associate Degree from accredited school in Social Work or another human services field is preferred .
  • Must be knowledgeable of social services practices, procedures, laws, regulations and guidelines governing long-term care .
  • Previous experience in long-term care is preferred.
