JOB SUMMARY:

The Director of Social Services - Licensed is responsible for providing social services to patients of the comprehensive outpatient rehabilitation program. They evaluate patients and identify need areas by coordinating and directing other social workers in carrying out their job duties. Additionally, coordinates services with the interdisciplinary team and contacts community resources as needed. The Director of Social Services - Licensed oversees case management services for patients and reviews patient status. The position is supervised directly by the Administrator and does administratively supervise other positions when specified per center. Strictly follows policy and safeguards computer passwords and computer use authorities. Understands the importance of logging on and off the computer clinical record system and will consistently maintain all computer protocols.

DUTIES/RESPONSIBILITIES:

• Evaluate patient need for social services.

• Coordinate social services with patient, staff, family members, and significant others as applicable.

• Provide social services as specified in the patient plan of care and consistent with treatment goals.

• Perform case management or case coordination as needed for patients.

• Contact and coordinate patient utilization of community resources.

• Provide social services to patients in accordance with accepted standards of social work and all applicable federal, state, and local laws and regulations and interpretations thereof.

• Provide specialized training to other staff, family members, consultants, and other individuals as needed.

• Participate in clinical case conferences and in-service programs to share information with the interdisciplinary team.

• Maintain appropriate documentation in a prompt and comprehensive manner.

• Attend team meetings and other meetings as required.

• Perform other duties as needed.

REQUIRED SKILLS/ABILITIES:

• Maintain high standards of ethical conduct as accepted in the field of social work and the health care industry.

• Perform other related administrative, managerial, or technical duties as required or suggested.

• Keep abreast of changes in procedures and treatment and suggest changes in Patient Policies as necessary.

• Create and implements personal goals, objectives, and strategies designed to address ongoing personal development needs to keep up with the rapid changes in the health care industry.

• Regularly exercise discretion and independent judgment with respect to matters of significance.

• Ability to plan and adapt policies and procedures relating to social services

• Strong organizational skills.

• Demonstrate integrity, imagination, and initiative, showing appropriate manner and conduct.

• Ability to relate positively, effectively, and appropriately with residents, families, community members, volunteers and other facility staff.

• Possess special interest in, and a positive attitude about, working with long-term care residents and the elderly.

• Ability to read, writes, speak, and understand English.

• Meets all health requirements, as required by law.

• Demonstrates basic computer knowledge and ability with an aptitude to learn company applications; proficient with Microsoft Office Suite.

EDUCATION and EXPERIENCE:

• Bachelor's Degree in social work or social welfare from an accredited academic institution required.

• Master's Degree preferred.

• Possession of a current state license as a Social Worker.

• Experience in outpatient medical rehabilitation preferred.

ENVIRONMENTAL AND PHYSICAL REQUIREMENTS:

• Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, ability to distinguish smells and temperatures, all with or without the aid of mechanical devices is required.

• Limited potential for exposure to environmental hazards. Understanding and adherence to company safety standards and protocols required.

• Meet all health requirements as needed.
