Accesses community and financial resources for participants and ensures continuity of care across the continuum. Conducts social work assessment and intervention in other care settings, including the participant's home, as needed. Roughly 60% out in the field seeing patients either in their home, AFH, ALF, or SNF's and 40% onsite in the clinic, but can vary depending on case load.
