Dignity Health Connected Living has been serving seniors in Shasta County since 1968. Our programs have been designed for seniors and people with disabilities. We are dedicated to meeting each participants physical mental and spiritual needs and we offer a variety of high-quality affordable health care services in a compassionate environment.

Responsibilities

Position Summary:

Under direct supervision the incumbent identifies and evaluates potential clients as total persons and identifies the functional limitations which impede independent living. This position reports to the Supervisor Care Manager.

Qualifications

Position Summary:

• 1 year experience working with the elderly and individuals with disabilities
  
• Masters degree in social work psychology counseling rehabilitation gerontology or sociology or a bachelors degree in one of the above fields and two (2) years experience
  
• Ability to relate to clients and families
  
• Excellent communication and presentation skills
