Job Requirements:

  • Prepared MSW or LCSW required. Past oncology experience preferred. 1 year acute hospital experience preferred. No travel requirements.

Role and Responsibilities

The Social Worker will deal with the timely assessment of psychosocial issues of patients and their families/support systems while advocating/coordinating a discharge plan and ensuring concurrent appropriate utilization of hospital services, coupled with available community services. Areas such as, but not limited to, medication assistance, living wills, health proxy's, available literature and resources in the community will be covered as needs are identified. They will maintain knowledge concerning regulatory standards, ex. JCAHO, ACHA etc., and will assist with keeping the department compliant with these standards. They will provide assistance and expertise with disease support groups and programs within the hospital as needed. Recognizes the signs and symptoms of stroke and calling a stroke alert overhead. Verbalizes stroke alert process. Works with the stroke patient and family for discharge planning and treatment based on needs, condition and prognosis of patient. Maintains currency in stroke knowledge by completing the annual 1 hour online training.
