NOW HIRING: Social Worker

Responsibilities Include:
  • Conduct comprehensive assessments of residents' social, emotional, and psychological needs.
  • Develop and implement individualized care plans in coordination with the interdisciplinary team.
  • Provide counseling and support to residents and their families regarding adjustment to the nursing home environment, end-of-life issues, and other concerns.
  • Advocate for residents' rights and ensure their needs are met in accordance with state and federal regulations.
  • Facilitate communication between residents, families, and healthcare providers to ensure the best possible outcomes.
  • Coordinate discharge planning and assist residents with transitioning to other levels of care when necessary.
  • Maintain accurate and up-to-date records in compliance with all legal and regulatory requirements.
  • Participate in team meetings, case conferences, and in-service training.

Qualifications:
  • Valid New Jersey Social Worker license (LSW or CSW).
  • Experience in a healthcare setting, preferably in a long-term care or nursing home environment.
